Annual Primary Residency Form

Form attesting you are using your Habitat home as your primary residence. You must submit this form once a year. We will send you this form via mail or email late summer of every year.

Annual Homeowner Survey

This form gives you an opportunity to share about your experience living in a Habitat home. We ask current homeowners to complete this survey once a year.
